In order to process a prescription we need your full name, patient number, medication name, strength and quantity required. Click this link to view the Prescription Form Layout.
The right hand side of the prescription form will show the date when your next medication review is due. Book an appointment to see the doctor around this indicated date. We may not issue you with your medication if an appointment for a medication review with the doctor is overdue.
It takes 48 hours to process a request for a repeat prescription.
All the local chemists have an ordering and collection service so you could contact them to pick up your prescription from the surgery and prepare it for collection at the chemist shop.
The chemist will sometimes drop the medication off at your home if you are too infirm to go and collect it or you don't have friends or family to fetch it for you.
If you are on regular medication it is important that you are seen by the doctor or nurse to ensure that the medications are working and are suiting you. Your medication review date will be printed on the right hand side of the prescription. Form time to time we may leave a message for you on the right hand side of the prescription.
We will not issue a prescription if you miss your check-up appointment.
